REGISTRATION FORMS
We would appreciate an up-to-date mobile phone number as we now use a system called MJOG which will send you a 'reminder' text message the day before you have an appointment booked.
MEDICAL QUESTIONNAIRE
We ask all our new patients to fill in a Medical Questionnaire, as it can take a considerable time for us to receive your medical records from your previous surgery. This form can be completed online. When you have finished the form simply click the send button. Alternatively you can print the form, complete it and bring it in with your pre-registration form.
Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
